How to Refer Patients to the Center
Healthcare providers, mental health clinicians, and other professional colleagues can support patients and families by sharing our contact information or by becoming a referral partner. Patients and families may call us directly or can be referred for services via an online portal for providers. This process allows professionals to connect their patients or clients to us directly and is designed to make accessing support easier at a time when the to-do list can become overwhelming.
Whether someone calls themselves or is referred by a provider, the first step in getting connected for services is a brief intake conversation with our Intake Coordinator, or our Bilingual Clinical Associate if in Spanish. This 15–20 minute phone call helps us learn about the individual’s situation and identify which programs and services will best meet their needs.
Referrals may include in-house programs or external resources when appropriate. We make every effort to answer calls promptly and return messages within one business day.
For clinical services such as counseling, child life, support groups, or nutrition counseling, individuals are connected to the appropriate clinician once enrollment and consent forms are completed. While occasional wait times may occur, most clients begin services within one to two weeks of referral.
If you would like to become a referral partner and gain access to our electronic referral system, please contact our Intake Coordinator. We can also provide brochures about our programs and services for you to share with patients and families.
