Advance Health Care Directives

Join Caroline Folkl and Shannon Hogan from St. Croix Hospice to learn about advanced care planning. They will discuss advance health care directives and the role of family, loved ones and healthcare providers in decision-making. Attendees will learn to evaluate personal values and wishes, potential healthcare concerns, and how treatment may impact quality of life.

Hybrid program. Refreshments and Snacks will be provided for those in person.
